Role Summary:
As a Medical Social Worker, you will play a vital role in providing psychosocial support and assistance to patients and their families to help them cope with the challenges of illness and navigate the healthcare system.
Responsibilities:
- Conduct psychosocial assessments of patients and their families to identify needs and concerns.
- Provide counseling and support to patients and families facing medical crises, chronic illnesses, or end-of-life care.
- Assist patients and families in accessing community resources and support services such as financial assistance, housing, and transportation.
- Advocate for patients’ rights and coordinate with healthcare team members to ensure holistic care.
- Maintain accurate and confidential patient records and documentation.
- Collaborate with interdisciplinary teams to develop and implement care plans and discharge plans.
Requirements:
- Bachelor’s or Master’s degree in Social Work or related field.
- Valid license or certification as a Social Worker.
- Proven experience working as a Medical Social Worker in a healthcare setting.
- Strong knowledge of psychosocial assessment techniques and intervention strategies.
- Excellent communication and interpersonal skills.
- Empathy, compassion, and sensitivity to the needs of patients and their families.
